• A death can be registered at the Embassy, provided the following conditions are satisfied:
  • The deceased is a citizen of Sri Lanka, and the death occurred in Jordan.
  • The declaration should be submitted within three (3) months of such death to the Embassy.
  • When an application is submitted after three (3) months and within one (01) year, an explanation should be provided to the Embassy explaining the delay.
  • When an application is submitted after one year, the consent of the Registrar General Department is required to register the death.
  • The declaration can be made by any one of the following:
    • The nearest relative of the deceased who is in Sri Lanka
    • A nearest relative of the deceased who resides in Jordan
  • A declaration form completed by the declarant (Form B 11, available for download here, or Form B 15, available for download here), along with documents to prove the identification of the declarant.
  • A valid Sri Lankan passport or any other available identification documents of the deceased to prove Sri Lankan citizenship.
  • A Death Certificate was issued by the Jordanian authorities for the deceased with an English translation.
  • If the cause of the death is not included in Death Certificate issued by the Jordanian authorities, it should be proved by supporting documents.
  • Original Birth Certificate of the deceased.
  • If the death has been registered in Sri Lanka, cargo receipts will be required.
  • Fee: JOD 16.000
  • If the declarant is living in Jordan and death occurs in Jordan, the declaration for registration of a death should be submitted to the Embassy of Sri Lanka in Jordan.
  • If it has not been registered at the Embassy and if the declarant is living in Sri Lanka, the declaration for registration of a death should be forwarded to the Central Record Room of the Registrar General’s Department.
  • Submission of forged documents and making false declarations is a punishable offence.
