General Information
- A Sri Lankan citizen can apply for a new passport.
- Ordinary passports are valid for 10 years from the date of issuance, unless otherwise indicated by any endorsement on the passport.
- Applicants are advised to apply for new passports in advance (at least six months prior to the expiratory date) with all required documents.
- On the expiration of 10 years from the date of issuance, if the passport was destroyed or lost, or there are no pages for visa endorsement, a new passport should be obtained.
- Submission of forged certificates or any other documents and making false declarations relating to citizenship status is an offence. Such cases will be reported to the relevant authorities and fees will not be refunded under any circumstances.
- Incomplete applications will be returned, and consular fees will not be refunded.

- The application should be filled out preferably, in BLOCK
- The applicant should place his/her signature without touching the lines of the two cages provided in the application.
- The new passport will be issued within 90 days after submission of the duly completed applications to the Department of Immigration and Emigration of Sri Lanka. However, the Department of Immigration and Emigration has the discretion to consider certain criteria (which include discrepancies in the photographs, the NIC number submitted, etc.) prior to the issuance of a travel document, and hence the time taken for the receipt of your passport is subject to vary.
- The applicant should physically be present at the Embassy along with the current passport to submit the application and to collect the new passport as well.
Documents Required
- Duly completed application form (Form ‘K’-35 A) (Download here).
- Three recent clear photographs, sized 3.5 x 4.5 cm. Please attach photographs with the full front face, both ears, and neck visible, without covering them by your hair, and avoid wearing spectacles and caps. Photographs should be taken within the last three months.
- Current passport.
- Photocopies of the data page of the current passport (Page Nos. 2 & 3).
- Photocopies of the observation and alteration pages of the current passport (Page Nos. 4 & 5).
- The original and photocopy of the certificate under the Citizenship Act (under Section 5(2) or 17) are required if the applicant was born outside Sri Lanka or is a citizen by registration.
- Affidavit for applicants who are born abroad and aged more than 21 years. (Download here).
- Original and photocopy of the Dual Citizenship Certificate (for those who have dual citizenship).
- Original and photocopy of the Marriage Certificate. (if the applicant is married and wishes to include/change the husband’s surname from her maiden name in the passport).
- Photocopies of the Sri Lankan National Identity Card, Birth Certificate, and Sri Lankan Driving License (if available)

- The original Birth Certificate of the child issued by the Government of Sri Lanka and its photo copy (translations are not accepted).
- If a child is born outside of Sri Lanka, the parents should provide the Consular Birth Certificate and its photocopy.
- The Citizenship Certificate or a copy of the payment receipt if the certificate is not issued at the time of application.

Note
- The child must be accompanied by both parents to the Embassy at the time of document submission.
- If both parents cannot be present at the time of the collection of the passport, one parent of the child should be present at the Embassy with the written consent of the absent parent for the issuance of the child’s passport.

- A Police Report from the Jordan Police stating that your passport is lost/stolen, along with an English translation.

NOTE
- The applicant should physically be present to submit the application at the Embassy and to collect the new passport as well.
- An Emergency Travel Document (Non-Machine-Readable Passport (NMRP) / Temporary Travel Document (TTD)), valid for six (06) months, may be issued to a Sri Lankan national, only to return to Sri Lanka in case of an emergency in the following circumstances:
- The original passport is lost/damaged/stolen.
- The original passport is expired.
- A Sri Lankan who does not have a passport or a travel document and wishes to return to Sri Lanka
- The applicants are required to call the Consular Division of this Mission in person for an interview.
- The applicants cannot travel to any other country by using a NMRP or TTD and cannot use it to obtain a visa to further stay in Jordan.

- Expired passport or damaged passport, if lost, a copy of the lost/damaged/stolen passport.
- A Police Report from the Jordan Police stating that your passport is lost/stolen, along with an English translation.
- Photocopies of Sri Lankan National Identity Card, Sri Lankan Driving License or a copy of the Birth Certificate.

Name Change
Documents Required
- A request letter from the person concerned to change the name in the passport.
- Duly completed application, Form O (I.M. 37) (Download here).
- Original passport.
- Photocopies of the data page of the current passport (Page Nos. 2 & 3).
- Photocopies of the observation and alteration pages of the current passport (Page Nos. 4 & 5).
- The original and a photocopy of the Marriage Certificate.
- The original and a photocopy of the Sri Lanka National Identity Card.
- The original and a photocopy of the Birth Certificate of the person concerned.

Profession Change
Documents Required
- A request letter from the person concerned to change the profession on the passport.
- Service letter mentioning the new designation on it.

- Photocopies of the observation and alteration pages of the current passport (Page Nos. 4 & 5).
- Educational, higher educational, and professional qualifications to prove the new profession.

Change the Status from the Middle East to All Countries
